Scholar Rock secured FDA approval for Isembyld (apitegromab), its first-ever product, for spinal muscular atrophy (SMA) in adults and children aged 2 and older who also receive SMN2-targeted therapies. The approval came two weeks ahead of the September 30 PDUFA deadline, following a prior rejection tied to a Novo Nordisk-owned fill-finish site in Bloomington, Indiana.
The phase 3 SAPPHIRE study showed a 2.2-point improvement on the Hammersmith Functional Motor Scale-Expanded versus SMN2 therapy alone, with 98% of patients continuing into the long-term extension study. Isembyld blocks myostatin to preserve muscle strength, and is set to ship in the coming days at a projected cost of $310,000 annually.
SMA affects roughly 10,000 children and adults in the U.S., and existing treatments from Novartis, Roche, and Biogen target the SMN2 gene. Isembyld is the first to directly target muscle atrophy, offering a complementary option for patients already on SMN2 therapies. For Scholar Rock, this approval transitions the company from clinical-stage to commercial-stage, impacting its operations, manufacturing partners, and market positioning.
